The date and place of baby's birth. If there is a multiple birth the time of birth will also be required.
About the Father
(when his details are to be entered in the Register)
His full name and surname at the time of the birth.
His date and place of birth.
His occupation at the time of the baby's birth.
About the Mother
Her full name and surname at the time of baby's birth.
Her maiden surname (if she is or has been married).
Her date and place of birth.
Her usual home address at the date of the birth.
Occupation (if she has been employed at any time before the birth this occupation can be entered if she wishes).
General Information*
The Registrar will ask if the parents were married to each other at the time of the birth, and the date of marriage.
You will be asked the number of children the mother has had previously by her present husband or by any former husband.
The Registrar will ask whether there are any previous stillborn children. *(These details are confidential. They will not be entered in the register and will not appear on any birth certificate).

A child will normally be given the surname of the mother or the father. Further advice is available from the Registrar if required.
Once the surname has been decided upon it may only be changed where:
The parents apply for a re-registration of the birth to have the father's details included when these do not appear in the first register entry
or
the parents marry each other at any time after the baby is born and registered. In this case the birth should be reregistered even if the child was given the father's surname at the original registration.
Where English is not the First Language
If English is not the first language of the parent(s) and they would like someone to help with the registration, they should ask a relative or friend to come with them to their appointment. It may also be helpful to bring the parents' passports and the birth certificates of any of their previous children.
Additional Copies of Birth Certificates
In addition to the first free short copy certificate further copies may be purchased from the Registrar at the time of registration or from the Register Office at a later date.
Please Remember One of the parents must register the birth personally. You can not ask a friend or relative to register on your behalf.
